Exclamation Royal Gramma Basslet has not grown, very lethargic, possible permanent caudal damage

My Royal Gramma has finally come out of the rock it has been hiding in after not seeing the fish for a week. It was very pale. The fish's caudal fin has never healed from damage dealt by Purple Dotty when all where acquired in February. It has also not grown any since I bought it, still about 1 1/4. When it moved from the rock where it had claimed to hide to a branch rock and between the glass, it no longer swam to eat. I put in hang on net breeder for a week. It would only move if I touched the net. I have not seen it eat in about a month. It still swims sideways and upside down, but in breeder net it would rest on bottom of net upside down.
So today I finally decided to put it in a full quarantine tank. It got some of its color back while I handled it to take pictures, but swam upside down before going up right in the QT. Then made home right in the corner of the glass and some rock I put in there.

I am going to get some Melafix tomorrow, but really don't know what to do. I will remove the LR before I treat.

You think you got the tank cleaned until you take a photo, see the water streaks on glass, algae still along the silicone.

It did not make it. I could not get it to eat.
